This report must be submitted to the Office of University Research Compliance within 48 hours of a laboratory biosafety incident.
CDC/APHIS Select Agents Official Forms
The transfer of select agents is a process that is highly centralized and controlled. The process in place at OSU has been approved by the Centers for Disease Control as part of the institution’s registration and approval to conduct activities using select agents. The Office of University Research Compliance is responsible for managing this process. Contact that office for information on transfer or receipt of select agents. Consult the section of this Website on select agents for more guidance.

The USDA and APHIS require this form for importing and exporting domestically and internationally. Additionally, APHIS requires that this form be submitted and the request be approved before a recipient entity can receive select agents. The individual investigator is responsible for ensuring that he/she has obtained this approval for the specific activity.

The FBI has specific requirements before approval is granted to persons working with select agents or persons having access to select agents.
